If you haven’t read about CSA options on the Tulip CSA page, please head there FIRST. Then please fill out this form and then mail or drop off a check for the appropriate amount to:

We do not accept credit card purchases either in person or online in order to avoid passing on credit card processing fees onto the customer.

If you are buying a CSA share or Punch Card as a gift, please fill out this form with your name and contact info, and put the information for the gift recipient in the “Is this a gift?” box.
